Medical Cannabis & Marijuana 329 Card Certifications

New patients, and renewals who saw other providers last time, need to see us in-person to qualify for our services. if you saw Susan Cauley, MD (our doctor) last time then we can renew your 329 card by telehealth. Only patients who saw Dr. Susan Cauley last time can obtain a 2-year renewal.

Tele-Health Process:

1) we will send you a link via text or email to register as our patient on the effective change llc clinical services platform. Please email to request instructions for telehealth renewals to matthew@doctor420hawaii.com and be sure to include your name, phone number, name of the doctor that you saw most recently, and where you are. We will reply promptly with the information you need to proceed.

2) you will create your basic client profile, and, using additional links that you get to your email address, you will complete all the administrative forms, and upload a photo of your id.

3) after you have completed your client profile then we will send you the payment invoice using paypal (other payment options are also available). Prepayment is required. Once prepayment is made, then you will be assigned to the clinician who will engage you on our encrypted tele-health video system.

4) you will need to create your state of hawaii dept. Of health (doh) application, here is the link for that:

5) you have the option for us to do all the dept. Of health online application work for you, for an additional fee (see more information, below). However, that work is no harder than what i have just described for you to register for tele-health, so you might as well save money as well as get your card sooner and do it yourself. Also, when you have your own doh account then you can download your card at any time, and also make changes more easily and for less money.

6) you will get an email from the doh about one week after you fo your online application with them, asking you to go to your account and download your card. It is not mailed to you.

Note: if you chose to have us provide full-service for you, then you must wait 3 weeks after the appointment for you to receive your card, and you get a hard-copy card in the mail; if you lose it then you must pay for a replacement.

Additional Services Offered

We do the application for you: $72, which includes the state fee $38.50 and our processing fee $33.50
If you see the doctor by telehealth then there is a $20 convenience fee for one year or two year (if applicable) renewals

Upload Id Only

We will upload your identification to the DOH on your behalf. You are responsible for filing the DOH application.

New patients: $202 cash/Venmo or add on 3% if using credit/debit card
Renew patients, in-person, one year: $182 cash/Venmo or add on 3% if using credit/debit card
Renew patients, telehealth, one year: $202 cash/venmo or add on 3% if using credit/debit card
Renew patients, in-person, two years: $220 cash/venmo or add on 3% if using credit/debit card
Renew patients, telehealth, two years: $240 cash/venmo or add on 3% if using debit/credit card

Change Form, Simple

We will do your change for you, which may include requesting a replacement card, new ID upload, address change, grow site change, or name change. $35 in person, $55 via Telehealth. Void 329 card, void caregiver, update contact information

Change Form, Complicated

We will add a caregiver, add or remove an "other" grow site, or do two or more changes at one time $40 in person, $60 via Telehealth

Note: If a patient sees us and does not register with the state within 6 months and then wants to do register finally the patient must come see the doctor and pay full price again

Counseling

Some patients need counseling to alleviate anxiety related to common issues surrounding cannabis production and use. Matthew Brittain, our Licensed Clinical Social Worker, offers counseling to reduce the anxiety, social stigma, legal consequences, and interpersonal complications that patients often experience.

Medical Expert Witness

We have been called upon on a number of occasions to provide medical expert witness testimony for patients involved in cannabis related legal challenges. If you, or someone you know, needs an expert legal witness in a cannabis/medical marijuana related case, please contact us.

Our forensic specialist in the cannabis field, Matthew Brittain, LCSW, provides expert witness services for court-involved patients.


Health Insurance

Health insurance is not accepted for the fees associated with the care coordination and medical services provided for 329 card certifications. Counseling sessions with Matthew Brittain are covered by most medical insurance programs. However, Kaiser, Medicare, and most Mainland insurance programs do not cover counseling.
